Official Rules — Aeternitas Foundation Membership Giveaway
NO PURCHASE NECESSARY. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED.
Entering this promotion constitutes full and unconditional acceptance of these Official Rules and of the decisions of the Sponsor, which are final and binding in all matters relating to this promotion. This promotion is a giveaway conducted by random drawing. No chances are sold and no payment is required or accepted for entry.
1. Sponsor
Sponsor: Aeternitas Health PLLC, 330 Billingsley Rd #211, Charlotte, NC 28211 (the “Sponsor”).
This promotion is administered solely by the Sponsor. It is not sponsored, endorsed, or administered by, and is not associated with, Instagram, Meta Platforms, Inc., or any other platform on which it is announced. By entering, each entrant releases Instagram, Meta Platforms, Inc., and every other platform on which this promotion appears from any and all liability in connection with this promotion. Questions about this promotion go to the Sponsor, not to any platform.
2. Eligibility
To enter you must, at the time of entry: (a) be 21 years of age or older; and (b) be a legal resident of the State of North Carolina.
The following are not eligible: the Sponsor’s owners, officers, physicians, employees, and independent contractors; the immediate family members and household members of each of them; and current Aeternitas members.
Limit one (1) entry per person across all entry methods, and no person may win more than one (1) prize. Duplicate entries, entries submitted by any automated or mechanical means, and entries submitted on another person’s behalf are void.
Age and residency are attested on the entry form. The Sponsor verifies eligibility before any prize is awarded. An entrant whose eligibility cannot be verified is disqualified.
3. Entry period
The entry period opens at 9:00 AM Eastern Time on August 15, 2026, and closes at 8:00 PM Eastern Time on August 15, 2026. The online entry form is available for the full entry period. The at-event entry form is available during event hours, approximately 11:00 AM to 3:00 PM Eastern Time. Entries received outside the entry period are void. The Sponsor’s clock is the official timekeeper.
4. How to enter — free
Entry is free. No purchase, payment, donation, deposit, reservation, or fee of any kind is required to enter or to win, and none will improve your chances of winning.
Enter by either method during the entry period:
- Online: complete the entry form at giveaway.aeternitas.health.
- At the event: complete the giveaway entry section of the event registration form at the Chilly Coffee Club event, held at a private venue in the Lake Norman area, North Carolina.
Both methods enter the same drawing with the same odds. The entry form asks for your name, email address, and telephone number, and your attestation that you are 21 or older and a legal resident of North Carolina. All of that information is required; an incomplete entry is void. If a person enters by more than one method, only one entry counts.
The $500 membership deposit is entirely separate from this giveaway. Paying it does not enter you or improve your odds; not paying it does not reduce your odds. No entry is conditioned on attending any event, presentation, or sales conversation.
5. Prizes
5.1 Grand prize
One (1) grand prize: one (1) year of Aeternitas Foundation membership. Approximate retail value: $7,000.
The grand prize consists of the following Foundation inclusions, fixed as of August 15, 2026, and not subject to change after entry: one deep-dive evaluation (including its standard diagnostic panel), a personalized action plan, a month-six check-in, one acute visit during the membership year, and four support-clinician visits (two therapist visits, one dietitian visit, one exercise physiologist visit).
The membership year begins on the date of the winner’s first scheduled visit, set by agreement between the winner and the Sponsor, and no later than November 30, 2026 unless the Sponsor agrees in writing to a later start. It runs twelve (12) months from that date.
The grand prize does not include: any service outside the inclusions listed above; any separately priced or physician-selected advanced diagnostic; the cost of any medication or supplement; travel; any second membership year; or any tax owed on the prize.
5.2 Secondary prizes
Three (3) secondary prizes: each is one (1) voucher for one DEXA body-composition scan and one VO2 max assessment at Aeternitas. Approximate retail value: $350 each.
Each voucher must be redeemed by March 31, 2027. DEXA scanning is expected to be available at the clinic beginning in September 2026. If availability moves later, the redemption window extends by the length of the delay and the Sponsor will notify each voucher winner in writing at the email address on the entry.
Each voucher service is subject to the Sponsor’s standard pre-assessment safety screening. If a screening finding makes one component clinically unsuitable for the winner (for example, pregnancy for DEXA scanning), or if one component cannot be made available, the Sponsor will substitute a service of equal or greater value for that component. If neither component can be made available, the Sponsor will substitute a service of equal or greater value for the voucher.
5.3 Terms applying to all prizes
No prize is transferable, assignable, or exchangeable, and no cash alternative is offered. The Sponsor reserves the right to substitute a prize of equal or greater value if an advertised prize becomes unavailable. Stated prize values are the Sponsor’s good-faith determination of fair market value. Prizes are delivered without charge and without any obligation on the winner.
Odds of winning depend on the total number of eligible entries received.
6. Winner selection
Four (4) winners are selected by random drawing from all eligible entries received during the entry period. The drawing is conducted on August 19, 2026 by Dr. Corey Helton, in the presence of at least one witness, using a computer-generated random selection from the deduplicated entry list. The grand prize is drawn first, then each secondary prize. A person already selected for a prize is removed from all later drawings. The entry list and a record of the drawing are preserved for at least one (1) year. Selection is final.
A person selected in the drawing is a potential winner only, subject to verification of eligibility under Section 7. No prize is won until verification is complete and the Sponsor confirms the award in writing.
7. Notification, verification, and claim
The Sponsor will attempt to notify each potential winner by email and telephone using the contact details on the entry.
Each potential winner must respond within seven (7) days of the first notification attempt and must sign and return an Affidavit of Eligibility and Liability Release within that same seven (7) day period. The potential grand prize winner must also: (a) return a completed IRS Form W-9, with the affidavit; and (b) complete the Sponsor’s standard clinical intake, as described in Section 8, within thirty (30) days of notification.
A potential winner forfeits the prize if they do not respond within the claim window, cannot be reached because the contact details on the entry are incorrect or undeliverable, are found ineligible, decline the prize, or do not return the required documents within the deadlines above.
On forfeiture, the Sponsor draws an alternate winner from the remaining eligible entries and applies this same process. The Sponsor will draw up to two (2) alternates per prize. If no eligible winner is confirmed after the last alternate, the prize goes unawarded.
8. Clinical intake and clinical suitability (grand prize only)
Foundation membership is medical care. The grand prize therefore carries one condition that entrants should understand before entering:
- Receiving the grand prize requires completing the Sponsor’s standard clinical intake — the same intake completed by any prospective Foundation member.
- Intake involves providing health information about yourself.
- Whether Foundation membership is clinically appropriate is determined by the supervising physician in the ordinary course of the practice’s standard intake, applying the same standard applied to any prospective member — not a standard created for this promotion. Foundation membership is clinically unsuitable only where the standard intake identifies one of the following: cancer currently under active treatment; cardiovascular disease that is unstable or that has required hospitalization or emergency evaluation within the six (6) months before intake; pregnancy; or a diagnosed condition whose current management plan requires ongoing specialist care that the Foundation tier does not provide. Any determination of unsuitability is documented in writing by the supervising physician.
- If the grand prize is determined not to be clinically appropriate for the potential winner, the membership is not awarded to them. In that case, that winner instead receives one DEXA and VO2 max voucher as described in Section 5.2 plus a five hundred dollar ($500) prepaid gift card (combined approximate retail value $850), delivered without charge and without any purchase or obligation, and an alternate winner is drawn for the membership under Section 7.
- Entering this promotion does not create a physician-patient relationship, and nothing in this promotion or in any material announcing it is medical advice.
- Health information collected during intake is handled under the Sponsor’s Notice of Privacy Practices and applicable law. It will not be used or disclosed for any marketing purpose.
9. Taxes
Each winner is solely responsible for all federal, state, and local taxes arising from a prize.
The Sponsor will report the fair market value of a prize to the Internal Revenue Service on Form 1099-MISC where the value meets or exceeds the federal reporting threshold for the year of award. The grand prize exceeds that threshold; a secondary prize on its own does not.
The grand prize winner must return a completed Form W-9 before the prize is released. A grand prize winner who does not furnish a valid taxpayer identification number is subject to 24% backup withholding; because the prize is services rather than cash, the Sponsor may require the withholding amount in cash before the prize is delivered. A W-9 not returned within the claim window forfeits the prize.
The grand prize is paid in services, not money, so its winner will owe tax in cash on value that does not arrive as cash. Prize value is also included in North Carolina taxable income. Winners should consult their own tax advisor.
10. Publicity and health information
Accepting a prize does not grant the Sponsor any right to use a winner’s name, likeness, image, voice, or statements.
With a winner’s separate written consent, the Sponsor may announce the winner publicly by first name only. Any use of a winner’s name, likeness, image, voice, or statements requires a separate signed authorization, specific to that use, which the winner is free to decline. Declining does not affect the prize.
The Sponsor will never publish any health information about a winner, in any form, and will not seek authorization to do so in connection with this promotion.
11. Privacy
Information collected on the entry form is used to administer this promotion — to verify eligibility, conduct the drawing, and contact winners. It is handled under the Sponsor’s privacy policy at https://aeternitas.health/privacy-policy/.
Entrants are not added to any mailing list as a condition of entry. Joining the mailing list is a separate, optional checkbox on the entry form, and declining it has no effect on your entry or your odds.
Entry information is retained until December 31, 2026 and is then deleted, except where the Sponsor must keep records longer to meet a tax or legal obligation.
12. Disqualification and the Sponsor’s rights
The Sponsor may disqualify any entrant who submits a false, incomplete, or fraudulent entry; submits more than one entry; enters by automated or mechanical means; tampers with the entry process or the drawing; does not meet the eligibility requirements in Section 2; or acts in a disruptive, threatening, or bad-faith manner toward the Sponsor, its staff, or other entrants.
If the promotion cannot be conducted as planned — including because of fraud, tampering, technical failure, or any other cause beyond the Sponsor’s reasonable control — the Sponsor may cancel, suspend, or modify the promotion and may award the prizes from among the eligible entries received before the interruption.
13. Limitation of liability
By entering, each entrant releases the Sponsor and its owners, physicians, employees, and contractors from any claim, liability, injury, loss, or damage arising from participation in this promotion, from acceptance of a prize, or from the use of a prize, to the fullest extent permitted by law. The Sponsor is not responsible for entries that are lost, late, misdirected, illegible, or not received, or for failed notification attempts caused by incorrect or undeliverable contact details on an entry.
Nothing in this section limits any liability that cannot lawfully be limited, and nothing in it alters the standard of medical care owed to any person who becomes a patient of the practice.
14. Governing law and venue
These Official Rules are governed by the laws of the State of North Carolina, without regard to conflict-of-laws principles. Any dispute is brought exclusively in the state or federal courts sitting in Mecklenburg County, North Carolina.

15. Winners list
To request the winners list, email info@aeternitas.health with the subject line “Giveaway Winners List,” or write to Aeternitas Health PLLC, Attn: Giveaway Winners List, 330 Billingsley Rd #211, Charlotte, NC 28211. Requests must be received by November 15, 2026. The list identifies, for each prize, the first name and city of the confirmed winner where that winner has consented to public identification; where a winner has declined, the list states that the prize was awarded to a winner who declined public identification.
